NOTICE OF APPEAL

NOW COMES Petitioner Juan Dela Cruz (“Petitioner”), through

the undersigned counsel, and to this Honorable Court, most
respectfully states that:

1. On 25 April 2020, Petitioner obtained a copy of the Order

dated 18 April 2020 rendered by this Honorable Court in the above-
entitled case (“Order”) denying the Motion for Reconsideration on the
Decision dated 29 March 2020 which denied the instant Petition.

2. Pursuant to Section 2 and 3 Rule 41 of the 1997

Revised Rules of Civil Procedure, Petitioner may file a notice of
appeal with this Honorable Court within fifteen (15) days from notice
of the judgment or final order appealed from or until 03 May 2020.

3. Petitioner hereby gives notice to this Honorable Court

that she is appealing the Decision of this Honorable Court with the
Court of Appeals. Petitioner attach to this Notice of Appeal proof of
full payment of the appellate court docket and other lawful fees.
