‘Abnormal temperature |s your temperature above 37.5°C or below 36°C? Generally unwell ‘Are you feeling generally unwell? For example flu like symptoms, shivers, chils? (even if you have a normal temperature) Chest pain CChest pain of any type must be treated as an emergency! Infection Do you have a sore throat, a cough or shortness ‘of breath, discomfort or burning when you pass urine, diarrhoea, a rash, bruising or bleeding for no apparent reason? Do you have a high or low temperature? Do you feel generally unwell? Has there been any shivering, chil or shaking episodes? 'Numbness/pins and needles, weakness in limbs When did the problem start? Is it continuous? Isit getting worse? Is it affecting activity function? Any constipation or unary incontinence? Activity levels/tiredness Has there been a recent change in activity or energy levels? Bleeding or Bruising You should be concerned if this is a new problem ‘ an old problem getting worse, Is the bleeding easily controlled? Has there been an injury? Diarrhoea ‘You need to measure any increase in your normal bowel movements (before you started treatment). Do you have any abdominal pain or discomfort? Feeling sick or being sick Have you taken your anti-sickness tablets? ‘Are you passing normal amounts of urine? ‘Sore mouth o you have any mouth ulcers or old sores? Is your mouth generally sore? Are there any signs of infection? Shortness of breath If this is an existing condition we want to know if t's getting worse. Pal new problem? Whereis it? How long has it been there? Have you taken any pain killers? Skin rash Is the rash widespread or in one small area? Is tchy? Are there any signs of infection? Constipation Consider your usual bowel movements. 4 no movement in 48 hours then please telephone for advice. Loss of appetite ‘Any other factors, for example diarrhoea, being sick or feeling sick, sore mouth or throat? Sore hands and feet 's your skin peeling? Do you have ulcers or blisters?
