 Original copy of duly accomplished Personal Data Sheet (PDS) (CSC Form
Number 212) (Revised 2017)
 Authenticated copy of Eligibility (issued by the CSC / PRC)
 Valid ID
 Two latest passport size ID pictures (white background, with signature over
printed name at the bottom part)
 Two Mongol 2 pencils
 Black pen
 Original and photocopy of the following:
 NSO-issued Birth Certificate
 NCIP-issued Height Waiver (if applicable)
 Official Transcript of Records
 College diploma
 Certificate of Eligibility or Report of Rating

ADDITIONAL REMINDERS:
 Applicants must be at the testing venue before 6:00 AM.
 Walk-in applicants will be accommodated provided they are qualified and
compliant with the abovementioned requirements.
 No fees will be collected.
 Applicants are generally notified by the PDEA Academy thru email and text / call.
 Age Waivers are not accepted.
 An applicant is highly discouraged from applying if he / she is not:
 Computer literate
 Of good moral background
 Physically fit
 Proficient in oral and written communication skills
 Willing to be assigned anywhere in the Philippines
 Willing to undergo rigid 6 months training
