By signing this document, I acknowledge that:

 The cost to participate in the La Vintage Bar & Grill’s Hari Raya Bazaar is RM 100 per day / RM 250 for 3
days with 50% deposit to secure booth space. (10ft x 10 ft Single Space + two (2) 2 ft x 4 ft display
tables + 2 chairs)
 Each vendor will be provided electrical outlet. (upon request)
 Booth space selection is by first registration first serve basis.
 Vendors’ operation time is from 9.00am to 8.00pm.
 Lucky draw will be held 2 times in a day. (vendors included in lucky draw)
 Free light refreshment buffet during breaking fast from 7.00pm until 8.00pm.
 Colouring Contest for Children. (vendor’s children can participate)
 Dateline for registration is by 6th June 2018.
 I have received the details on information of terms & conditions, and aware and shall comply with it.
