AFFIDAVIT

WE, GUILBERT JAGONAL and GLENDA E. JAGONAL, all of legal

age, Filipino, married, and a resident of Zone 15 Poblacion, Manticao Misamis
Oriental, after having been duly sworn to in accordance with law, depose and state:

1. That we are the operator of Sr. Pedro “Lechonan” Lugait branch at Masilakon 1,
Poblacion Lugait, Misamis Oriental;

2. That the said aforementioned “Lechonan” is fully operated by the abovementioned

operators;

3. That We execute this Affidavit to attest and declare the truth of the foregoing facts
and for whatever legal purposes that this Affidavit may serve.
