RE: Dam Safty Inspection of the ANDREA DETENSION BASIN #3, J-502, NV10819
‘To Whom It May Concern:
‘On May 15, 2018, Melissa Marr and Amanda Brownlee, PE. ofthe State Engineer's office,
inspected the D’ANDREA DETENSION BASIN #3. The purpose ofthe inspection was to
determine the condition of the structure with respect to dam safety,
‘The DANDREA DETENSION BASIN #3 i classified as a High hazard dam due to downstream
development. As this dam is classified asa significant or high hazard dam, an Emergency Action
Plan (EAP) must be developed, exercised and updated regularly. Per State Engineer Order 1294,
which was deted January 16,2018, an EAP was to be submitted to this office within 60 days
To date a EAP has not been submitted to this office and this matter has been handed over to
the Compiance and Enforcement section for further action, This letter i addressing the visual
inspection only
‘The overall condition of the dam is FAIR. Pleace note that this physical inopection may not
‘deni all potential risks wit this dam and the inspection report i nt intended vo be an
valuation ofthe proper operation and management ofthe dam. The dam owner is encouraged
to seck qulfed resources to ensure proper maintenance; regulatory compliance and dam safety
‘management are carried out. The owner i ultimately responsible forthe safe operation and
Performance of their dam. A dam owner should also be familar withthe legal obligation to
‘maintain and operate the dam in a safe condition. Based on the engineers observations, the
following corective actions should be taken:
Short Term (1 Year)
|. INLET TO RESERVOIR - Debris: Remove all obstructions debris from the spillway.
Photos Attached,Long Term (2 Years)
1. ALL DAM- Rodent Activity: Rodent activity should be monitored and any holes
{lseovered should be backfilled and properly compacted. A rodent contol program should
be instituted, - Photos Attached.
2. RIGHT UPSTREAM ABUTMENTIGROIN - Other: Antils should be monitored and filled
{nto prevent water from traveling into dam structure - Photos Attached.
‘The above items are considered maintenance issues and donot require a new approval of plans
snd specifications (permit to accomplish. Please be aware tha future modifications may eure
submittal of plans and specifications for review and approval (permit). Additional infurteticn on
