AFFIDAVIT OF SERVICE

I, Ronald E. Amasan, of legal age, married and resident of Basak, Tamiya, Lapu-Lapu City, after
having been duly sworn in accordance with law, hereby depose and state that:

1. I am a collector of Cebu Sendai Motor Sales, Inc. (hereafter, the “Company”) assigned
at the Company’s ______. As such collector, I am tasked to personally visit the
Company’s customers to collect their installment payments as well as to personally serve
upon them such notices and letters, including demand letters issued to them.

2. One of the customers of the Company is Mr. ___________ (hereafter, “_____”) whose
account became delinquent starting ________.

3. Thus, in order to notify him of such fact, our Credit and Collection Supervisor, Mr.
Marvin Lance S. Ortiz, prepared a demand letter addressed to him dated _________. In
order to personally serve the same, I scheduled a trip to visit Jose at his residence at
________ last _______.

4. When I served the said demand letter, it was Jose who personally received the same. I
am certain of this since I have met him even before (Date of service of the demand letter)
and I am certain that he was the same person to whom I served the demand letter. Before
handling to him said demand letter, I even talked and asked him to settle his obligations
with the Company.

5. As a matter of procedure, I requested ____ to sign on the receiving copy of the letter.
Such demand letter was personally received by ___ as signified by his signature at the
bottom left of said demand letter.

6. Thereafter, I left the original of the demand letter to ___ and brought back to the office
the receiving copy thereof.

7. I execute this Affidavit to attest to the truth of the foregoing and for whatever legal
purpose it may serve.
