CERTIFICATION OF CSO EXISTENCE

This is to certify that:

 MAGPAPAWID SLP ASSOCIATION, with business address at Sitio Tarabanan, Barangay

Concepcion, Puerto Princesa City has been organized by SUSTAINABLE LIVELIHOOD
PROGRAM, a program designed to alleviate poverty through livelihood programs
and micro-enterprise development projects.

 The said CSO has satisfactorily complied with and accomplished the requirements
and processes required in the accreditation guidelines;

 The CSO has supplied information found to be both consistent and correct in its
initial proposal and verified during the subsequent field validation;

 An ocular inspection/site visit has been previously done and the said CSO is found to
be existing and currently operating since June 15, 2018.

In compliance with DSWD MC 17 Series of 2017, this certification is issued this ____ day of
