INTRODUCTION
- SECTION 1
This privacy notice explains why Dixit Womancare Ltd collects information about you, and how that
information may be used.
The General Data Protection Regulation (GDPR) Article 29 is a regulation in EU law on data protection and
privacy for all individuals within the European Union which comes into force on 25th May 2018. It enhances
existing legislation that we currently follow.
HOW WE USE YOUR INFORMATION

- SECTION 2
The health care professionals who provide you with medical care maintain records about your health and any
treatment or care you have received previously (e.g. NHS Trust, GP Surgery, Walk-in clinic, etc.). These
records help to provide you with the best possible healthcare.
Your health records may be electronic, on paper or a mixture of both, and we use a combination of working
practices and technology to ensure that your information is kept confidential and secure. Records which Dixit
Womancare Ltd may hold about you may include the following information:
(A) Information collected and held is as follows:
• Your name
• Your date of birth
• Your next of kin
• Your address
• Your email address
• Your contact number(s)
• Your GP address
• Your insurance details
(B) Further information in respect to individuals that have seen Ms. Dixit previously or may see Ms. Dixit
again is as follows:
• Clinical letters – dictated and typed at the time of appointments and procedures
• Medical reports i.e. scans, bloods, lab results etc – sent to us via a secure fax or via post to Ms.
Dixit’s registered address
• Detailed medical history and consultation notes taken by Ms Dixit at your consultation and held
securely in her office.
• Relevant information from other health professionals, relatives or those who care for you

OBTAINING YOUR CONSENT AND UNDERSTANDING

- SECTION 3
We will rely on your consent to process the information set out in A/B and overall throughout this policy
which is collected at the outset of your registration and that time, you will receive a text message on the
mobile phone number you provided from the Private Secretary asking the following:
Following your consultation(s), clinic letter(s) may be dictated & you may undergo tests therefore (in line
with GDPR), we want to ensure you are happy to receive such things via the email address you provided
at the time of registration.
If you provided us with your GP’s details, it is common practice for us to keep your GP copied into your
clinic letter(s) and tests – if you do not wish for this to take place, please state.
Please can you confirm that you accept the above and if so, to add, your clinic letter(s) will be always be
PASSWORD PROTECTED (PDF formatted) with details of the password texted to you. Please keep this
text message safe.
Alternatively, if you would like other arrangements made and/or do not wish to receive such information,
please again state.
HOW WE HOLD YOUR INFORMATION

- SECTION 4
All the personal data we have is stored on our secure database, protected with encryption via keychain on
an iMac. Handling and processing this medical information means we are acting as the “data controller” for
the purpose of providing medical care. Your records are used to ensure you receive the best possible care.
We are committed to protecting your privacy and will only use information collected lawfully in accordance
with the Data Protection Act 1998 (which is overseen by the Information Commissioner’s Office), Human
Rights Act, the Common Law Duty of Confidentiality, and the NHS Codes of Confidentiality and Security.
We maintain our duty of confidentiality to you at all times. We will only ever use or pass on information about
you if others involved in your care have a genuine need for it. We will not disclose your information to any
third party without your permission unless there are exceptional circumstances (i.e. life or death situations),
or where the law requires information to be passed on.
Your data will be retained for no longer than is necessary but in accordance with the legal obligations we are
required to meet.
WHO ARE OUR PARTNER ORGINSATIONS?

We may also have to share your information, subject to strict agreements on how it will be used, with the
following organisations:
• NHS Trusts
• Specialist Trusts
• Independent Contractors such as dentists, opticians, pharmacists
• Private Sector Providers

For us, information corresponding to yourself will also come via a third-party Hospital whom we work
alongside/at such as The Portland Hospital, BMI Syon Clinic, BUPA Cromwell Hospital, BMI Clementine
Churchill Hospital or BMI Princess Margaret Hospital. If you would like any of the following Hospitals / health
care providers’ policies outlining their practices in line with GDPR – we suggest making contact with the
Hospital directly yourself.
ACCESS TO PERSONAL INFORMATION

- SECTION 6
You have a right under the Data Protection Act 1998 to access/view what information the surgery holds about
you, and to have it amended or removed should it be inaccurate. This is known as ‘the right of subject access’.
If we do hold information about you, we will:
• give you a description of it

• tell you why we are holding it
• tell you who it could be disclosed to
• let you have a copy of the information in an intelligible form
However, if you are consenting to the above via text message (section 3) through the Private Secretary, you
will always be in receipt of your medical information by email/post
YOUR RIGHT TO OPT-OUT

- SECTION 7
You can decide to opt-out of receiving text messages and emails from the practice and/or sharing
information with secondary care organisations. However, this may restrict the service we can provide you.
Please contact the practice if you require further information.
CONCERNS
- SECTION 8
If you have a concern about the way we are collecting or using your personal data, you should raise your
concern with us in the first instance or directly to Information Commissioners Office.
