PROTEST

hereby state, that:

I have been duly appointed as a watcher in Precinct No. _____ of

Barangay ___________, Municipality of Opol, Misamis Oriental for Mr.
Maximino B. Seno, who is a candidate for Mayor of Municipality of Opol;

the Board of Election Inspectors of Precinct No. _____, Barangay
________, Municipality of Opol;

He/She is known to be a relative within the fourth civil degree of

affinity or consanguinity of __________________________________, who
is a:
(1) candidate for ______________________ of the Municipality of
Opol for the May 9 2016 Elections; OR
(2) member of the Board of Election Inspectors;

Being a relative within the fourth civil degree of affinity or

consanguinity, he/she has violated Section 6 of COMELEC RESOLUTION
NO. 10031 and Section 166, Article XIV of the Omnibus Election Code,
which provides that no person shall serve as a chairperson or member of
the Board of Election Inspectors if related within the 4th civil degree of
consanguinity or affinity to any member of the Board of Election Inspectors
or to any candidate to be voted for in the polling place or the candidates
spouse;

I am executing this protest/complaint to attest to the truth of the

foregoing, to disqualify and exclude ________________________ as
___________________ of the Board of Election Inspectors of Precinct No.
_____, Barangay ___________, Municipality of Opol, Misamis Oriental and
for whatever legal purpose this may deem serve.
