PROFESSIONAL REGULATION COMMISSION

LICENSURE EXAMINATION & REGISTRATION INFORMATION SYSTEM (LERIS V2)

APPLICATION FOR EXAMINATION


1. Before you start, you must have the following on hand:

- Your e-mail address (with password)

- Your mobile number (ex. 09271234567)

- Personal data (including date of graduation)

- ID photo in .jpg format saved on your computer, phone or tablet.


(You can request for an e-file if you have your ID photo taken at a
photo studio.)

- Cedula (residence certificate)


2. Visit the PRC website at www.prc.gov.ph and point the mouse cursor to the Online
Application then click where you want to apply: Central/Regional Office, Robinsons, etc.
3. If you are using this for the first time, click on the REGISTER tab and fill out the form. You
MUST create your OWN account using your OWN email address. You are allowed to use
ONLY ONE VALID EMAIL ACCOUNT PER USER/APPLICANT. Complete all fields using
drop down menu where applicable then choose ACCEPT TERMS OF SERVICE.
3A. Read the Terms of Service CAREFULLY and click on the green button if you wish to
continue.
3B. Click the SIGN UP NOW button.
4. To activate your account, access your registered e-mail account, open the PRC generated
email then click Activate Account to verify your account.
5. If you have previously registered online or finished signing up, enter your email address and
password, then click SIGN IN.
6. Create your profile. To make sure that information is accurate, you must fill up these forms
yourself. Make sure the First & Last name is exactly the same as printed on your existing
PRC ID. Where appears, search with the drop down menu. Do not type in the
information.

7. After filling out your profile. Click on the SAVE INFORMATION button..
8. Double check your information before you click on the YES button. (You will not be allowed to
change your name after clicking YES.)
9. Your application details will be displayed. You may still edit your information (except for your
name) by clicking on the EDIT button. Click the icon to upload your photo.

10. READ the photo requirements carefully. To avoid delay or disapproval, please upload good
quality ID photo. Click the OKAY button to proceed.
11. Upload your picture via your computer or phone directory.
(Photo size must not be more than 300 x 300 dimension. If photo is not in accordance w/
specifications, you will be required to change the photo you attached.)
12. To continue, click SELECT TRANSACTION.

13. Select EXAMINATION.

14. Select the Name of Examination, Examination Type, Date of Examination and Place of
Examination, then click the PROCEED button.

15. Select where you want to apply (ex: do not choose Baguio if you intend to apply at the
Central Office. Choose an appointment date (only from colored boxes). Then choose your
preferred payment option.
(If you choose any of these payment options, there will be a service fee of 45 pesos, For
some payment channels may charge an additional fee on top of the 45 pesos service charge)

16. After choosing a payment option, a confirmation window will appear with a Reference
Number. COPY YOUR REFERENCE NUMBER AND PAY AT YOUR SELECTED PAYMENT
CHANNEL.
17. AFTER PAYMENT, verify your status at EXISTING TRANSACTION Tab. Select PRINT
APPLICATION FORM button.
18. Click to print the Application Form. Then click the CLOSE button.

19. Click your name on the upper right corner of the screen and click the LOGOUT button.

REMINDER
Remember: always LOG OUT at the end of your online session especially when using public
computers (Internet and cyber cafes). This is to prevent others from accessing your account and
to protect yourself from fraud, identity theft and other malicious intent.

20. Proceed to the PRC Office you selected in the appointment
page. Bring all required documents.
