On behalf of my research groupmates, I, John Paulo B. Tuazon, a Bachelor of Science in Civil

Engineering student, would like to ask permission to your good office to allow us to use and borrow the
Cylindrical Mold as a valuable equipment in material sampling for our research study entitled “Use of
Waste Paper as a Partial Replacement of Cement-Sand Mixture in Non-Bearing Partition Panel” this
coming August 30, 2018.

In line with this, we are requesting your good office to let us borrow and use the following engineering
equipment:

• 6 Cylindrical Mold
• Metal Rod
