TO WHOM IT MAY CONCERN:

This is to certify that Ms. Juana Dela Cruz, 25 years old, with
residence at __________________, was examined and treated in the
medical examination facilities at the NBI headquarters in Manila on
29 May 2018 with the following findings:

- deep lacerations at the 9 o’clock position within the vaginal

wall due to insertion of a blunt cylindrical item
- with emission and traces of semen found in uterine lining
- hematoma at the neck

Issued this 6th day of June 2018 with appendices, for whatever
legal purpose it may serve.
