BSA OFF-CAMPUS WORK EXPERIENCE PROGRAM

MEMORANDUM OF AGREEMENT

This agreement is entered into by the _____________________________, Davao City,


hereinafter referred to as the FIRST PARTY, ___________________________and with office
address at ____________ _, represented by hereinafter referred to as the SECOND PARTY.

It is hereby agreed that:

1. The Second Party will permit _______________________ to enter its place of business
or workplace for the off-campus training of the student in accounting and/or auditing
related work.

2. The required length of time of the off-campus practicum is a minimum period of 200
hours that will be conducted 4 hours per day, Monday to Friday. The student shall be
constantly under the supervision of the person-in-charge designated by the Second
Party and the instructor-in-charge designated by the First Party.

3. The status of the student while in training shall be that of the student-trainee and not of
the employee-employer relationship.

4. The student, while in training, shall progress from job to job in order to gain exposure
and experience in all phases of operations and all aspects of duties.

5. The student, while in training, shall observe the rules and regulations of the Second
Party and shall abide with all implied and stated terms of this Memorandum of
Agreement.

6. It is the responsibility of the student-trainee to make every effort to attend regularly the
off-campus practicum. The student-trainee shall notify the Second Party in advance in
case an unavoidable absence from the training will be made. In cases of prolonged
unavoidable absences, the student-trainee should notify the designated persons-in-
charge of the First Party and Second Party.

7. The Second Party will make a report with regards the performance of work, attitude,
and regularity of attendance of the student-trainee to the First Party.

8. The WAIVER OF CLAIM which is hereto attached becomes a part and parcel of this
Memorandum of Agreement.
