Upon learring ofthe situation, Sacred Heart Rehabiltaion Center promplly launched an investigation and engaged extemal cersecurty professionals, ‘Alter devoting considerable time and resources to determine what exact information was contained in te affected employee's email account, Sacred Heart Rehabiltaion Center discovered ‘on November 16, 2016 thatthe email account contained certain patients’ information, including ful names, addresses, health Insurance information, medical treatment information, medical diagnostic Information andlor Social Security numbers. This incident dd not affect al patients Sacred Heart Rehabilitation Center is rot aware of any reports of identity fraud or improper ‘se of information asa direct result ofthis incient. Sacred Heart Rehabilitation Center has mailed letters today to patients whose senstve personal and healt information was contained inthe affected ermal account ‘The patents whose Social Secutty nunbers were contained inthe account may enrol in a edt monitoring and identy thet restoration service, which is being offered al no cost. Those patents have also been provided with best prectcas to protect ther information, including steps to ‘obiain a free credit report, placing a fraud alert andlor placing @ secur freeze on ther cre files, “Those patients have also been reminded to renin vigilant in reviwing financial account statements ‘ona reguar basis fr any fraudulent activity. Is also recommended that affected patients review the statements that they receive from thelr health insurance providers and follow up on any items not recognized ‘Sacred Heart Rehabilation Center has ‘aken steps to minimize the sk of similar incident in ‘the future, including implementation of aditonal ployee traning and securly measures.
