I have been selected as an All-State Chorus student from______________________ High School.

In the event that I am no longer enrolled in the afore mentioned High School or enrolled in a choral program at
said High School, I forfeit my right to audition and participate in the All-State Chorus. I agree to abide by the
rules governing the 2019 All-State Chorus. I agree to abide by my school related activity policies. I agree to
accept full responsibility for any behavior deemed unacceptable by the All-State Chair and my director. Failure
to comply will result in immediate expulsion from the 2019 All-State Chorus at the parents’ expense.

I hereby agree that WVMEA has unlimited rights to use any audio/video recording and
photographic images of my child’s participation and performance at the WVMEA
conference.
