I refer to your recent enquiry regarding the non receipt of your NSW Drivers Licence

In order for us to process a replacement licence, you will need to sign the below declaration. You
can fax this back on +61 2 8059 4470 scan and email to photo.kit.unit@rms.nsw.gov.au or mail it
back to the address as shown below.

Please note this declaration must be received by no later than the date stated in the email. If your
non-receipt declaration is received after this date you will be required to submit a new photo kit
application and pay any associated fees.

To guarantee your signed declaration is received by this date, please phone our photo kit unit on
02 8894 1599 once you have submitted the document for processing to ensure your declaration
has been received.
