Instructions:

Step 1- Check on top tool bar that you have the developer tab. You will need this tab to edit.

Step 2- If you do not have the tab, open Word menu click “Preferences” and under “Authoring and Proofing Tools”, click “View”… Then under “Ribbon” check the
“Show Developer Tab” box. (Ignore this step if you already have developer tab).

Step 3- Edit according to company preferences/needs.

Step 4- When finished editing, select “Protect Form” from developer tab. This allows you to check the boxes but halts editing option.

Step 5- Now your check list is ready to distribute!
