Schizoaffective disorder

Epidemiology
• Prevalence: 0.05%
• Gender: F > M

DSM 5 Criteria
Presence of
1. Only hallucination or delusion for at least 2 weeks, in absence of affective episode throughout
psychiatric illness
2. Continuous period of significant affective symptoms at the same time with symptoms of
schizophrenia
3. Must have symptoms which fulfill diagnosis of affective (mood) disorder throughout psychiatric illness

• Bipolar type: When manic episode happened throughout psychiatric illness


• Depressive type: When major depressive episode happened throughout psychiatric illness

Investigation
• Same as schizophrenia

Treatment

Condition Treatment
1. Psychotic symptoms • Anti-psychotic (Atypical) (Olanzapine)
2. Manic symptoms • Mood stabilizer (Lithium or carbamazepine)
3. Depressive symptoms • Anti-depressant (SSRI Most common)
4. Not response to medication • Electroconvulsive therapy (ECT)
5. Psychosocial treatment

Prognosis
• Intermediate between schizophrenia & affective disorder
• Manic subtype, better prognosis
