JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

resident of Brgy. Sta. Lucia, Capas, Tarlac, and _________________________, of
legal age, ___________, Filipino, and a resident of Brgy. Sta. Lucia, Capas,
Tarlac, after having been duly sworn to in accordance with law, hereby depose
and state: That-

1. We personally know JOSE LAYSON, JR. who is a family friend and an

acquaintance;

2. We personally know that he was born to spouses JOSE AQUINO

LAYSON, SR. and TEOFILA GARCIA GUECO on May 26, 1946 in Capas,
Tarlac;

3. We personally know that when TEOFILA GARCIA GUECO gave birth to

JOSE LAYSON, JR., it was attended by a traditional midwife;

4. We personally know that by reason of such fact, TEOFILA GARCIA

GUECO forgot to cause the registration of the birth JOSE LAYSON, JR.;

5. We are not related to JOSE LAYSON, JR. either by affinity or

consanguinity;

6. This affidavit is being executed to attest to the truth of all the foregoing
facts and for whatever legal intent and purposes it may serve.
