DESCRIPTION BRAKE BAND FAILURE

CIRCUMSTANCES / EVENTS
On Oct 26, 2014 @ 2000hrs, the crew was drilling and was @ 3582m. The driller was hoisting the drill string before the next
connection, when he went to apply the brakes and the brake handle fell down to his knee level. At that point there was no one
on the floor except the driller, he immediately stopped the drill string and chained down the brake handle. He notified the Rig
Manager, who immediately came to the floor and the crew had removed the guards from the drawworks, upon inspection he
observed a broken brake band on the off-drillers side and driller side was cracked. Rig Manager immediately notified the Field
Superindentent, and new brake bands were dispatched by hot shot to location. During that time the crew removed the old brake
bands, and prepared the drawworks to reinstall the new brake bands. Once the new bands arrived they were installed on the
drawworks and tested and adjusted before continuing operations. During the remainder of shift, the crew checked the bands
every hour to ensure everything was ok. New certifications were received the next morning for the brake bands.

 Structural failure of the brake band material causing cracking, and eventual breaking.
ROOT CAUSE  Additional results pending 3rd party inspection.

CORRECTIVE ACTION ASSIGNED TO


1) Continue weekly & pre-spud inspection of braking system and drawworks. Drillers should
notify Rig Managers of any abnormal sounds or characteristics of the hoisting equipment.
1) ALL RIGS
Rig Managers must ensure reports on abnormal operating conditions are investigated
thoroughly and reported to Field Superintendent.
