Subject: A certain Incident at front desk around 7.45pm-8.30pm at 30/01/2019.

Dear Sir,

I am Subir Saha, ID 2034 working at your organization from last 5th November, 2018. I always
try to maintain office curriculum as part of your organization. Last day around 7.45pm-8.30pm at
30/01/2019 happened an incident at front desk. Last day I, Mr. Asif & Ms. Shabnom did evening
duty at help desk. There were too much crowed at help desk on the mention time. Ms. Shabnom
was at registration desk, I was in front of desk to counseling with all patients, and Mr. Asif was
gone to prayer. In that time there was 5-6 patients registration paper at Shabnom hand, because
she was personal talking with a patients. Then a told her please do fast patients registration. She
did not obey to me. Sometimes later Mr. Asif came at desk after his praying. Then due to that
current circumstance I went inside to do some registration so that all patients can get a fast
service. I was doing registration and as well as counseling with the patients. But suddenly Ms.
Shabnom shouting with me so why I was doing registration process in front of Patients and all
others. I told her please talk with me later about this matter did not shouting in front of guest
patients. In that time I was doing registration MST NASRIN AKTER phone 01763321113 staff
of BSH. Then she left the desk and go to in charge room. After sometime she came back and told
me I don’t know any manner, my educational background is not qualify, even she threat to me
that I will see you how you do job at BSH. Then I reply her words but I never use any unethical
language.

But she complained about me I used some bad words to her when I talked with her. Please it’s
a humble request to you please check CC camera footage to see the yesterday situation
because she had left desk 2 time for her personal purpose.
