AFFIDAVIT OF LOSS

I, JUNEL JAY C. CAÑETE, Filipino, of legal age, and a resident of 010

Hillside, Mangahan, Lower Balulang, Cagayan de Oro City, upon my oath, do
hereby depose and state - That:

1. I am a Grade 9 student of Balulang National High School and the holder of

a Grade 9 Panitikang Asyano Textbook.

2. On January 2018, without any negligence on my part, the Panitikang

Asyano Textbook were misplaced in the classroom.

3. I have exerted diligent efforts to search for the said Panitikang Asyano
Textbook books, but my efforts proved in vain.

4. For legal intents and purposes, the lost books are beyond recovery.

5. I am executing this affidavit to attest to the loss of the aforementioned

and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I hereunto affix my signature this March 26,
