Communication Skills For Chronic Illness

Definition :

Therapeutic communication is a consciously planned and purposed communication and


its activities are focused on the patient's healing, and is a professional communication that leads to
the goal of healing the patient

Some opinions about therapeutic communication include:

1. Northouse (1998) defines therapeutic communication is as a nurse's ability or skill to help


clients adapt to stress, overcome psychological disorders and learn how to relate to others.

2. Stuart G.W (1998) states that therapeutic communication is an interpersonal relationship


between nurses and clients, in this connection nurses and clients gain shared learning experience
in order to improve the client's emotional experience.

Chronic illness is a disease that the course of the disease lasts for many years, increases
weight, persists and often relapses. (Purwaningsih dan Karbina, 2009)
CONTENT
Delivering bad news
The steps are :

a. Preparation
Understand yourself as a nurse and prepare yourself with a variety of information.
The best thing in delivering bad news is by meeting directly with the person we are going
to. Convey unclear and frightening should be avoided like: "Mrs. Sri, come soon, I have
something I have to say to you".
Besides, it would be better if the nurse provides a seat for the nurse, the doctor
and the person to talk to, sit down and show that you are paying attention and not in a
hurry. Prevent talking while running or in inappropriate places e.g. hospital corridors that
many people.
Tell your colleagues that you can not be disturbed while you tell the patient the
news. Arrange the sound to make you look normal, not erogenous or vibrating.

b. Making connections
Make an initial conversation, even if you think that the person you are talking to
already has an inkling of what you are about to say.
Some important tasks at the beginning;
• Initial conversation
1. Introduce yourself and the person you are with, if there is a person in the know by the
nurse then find out who he is.
2. Assess the status of the recipient
3. Ask the news or comfort and needs. You should examine the recipient's understanding
of the situation. This will help the nurse in making the transition in delivering bad news
and will assist the nurse in assessing the patient's perception of the situation.

c. Share a story
There is an allegory that bad news is like a bomb. The radiation will be about all
the existing environment.
• Talk slowly
Give an early warning "I'm afraid I have some bad news for you ....
Sentences should be short and a few short sentences.

d. As a result of news
Wait for the reaction and calm down
For example: crying, fainting etc.
• View and respond as empathic
And the nurse can say "I understand, it is difficult for you. What's on your mind right
now?
• Follow and note the next recipient
You can help the recipient to master control by asking
"Do you need new information or can we talk later? "
Give attention and respect the nurse's feelings and needs
Often nurses feel heavy heart and feel stressed when retelling a bad news. Therefore,
sharing experiences and feelings towards colleagues is in need and can as a support
system for yourself.

IV. Conclusion

The nurse-client relationship therapeutic is a shared learning experience and client's


emotional refinement experience. In this case the nurse uses herself therapeutically by using
various communication techniques so that the client behavior changes to positive optimally. In
order for the nurse to play an effective and therapeutic role, he must analyze himself from self-
awareness, clarification of values, feelings and capable of being a responsible model. All
behaviors and messages conveyed by nurses (verbal or non verbal) should aim therapeutically
for the client.

Therapeutic intimate relationship analysis needs to be done to evaluate the development


of relationships and determine the right techniques and skills at each stage to address the client's
problems with the principles here and now (here and now). Safety is the main thing that children
should give children free to express his feelings without criticism and punishment.
