AFFIDAVIT
(of no other employment during tenure)

I, LESLIE JAY D. TAROY, 24 years old, married, Filipino and a

resident of Cogon, Gitagum, Misamis Oriental, after having sworn to in
accordance with law, do hereby depose and state:

1. That I was elected as Sangguniang Kabataan President of Barangay

Cogon, Gitagum, Misamis Oriental last 2010 SK Election and took an
oath for the said public office on the same year;

2. That I have served the said office for a period of three years or until
year 2013;

3. That during my tenure of service, I had never been employed in the

government nor received any form of salary or compensation by
reason thereof, except that of being an SK President of the above-
mentioned barangay;

4. That I am executing this affidavit to attest the truthfulness and

veracity of the foregoing statements and for the purpose of applying
the eligibility grant for public officials before the Civil Service
Commission.

IN WITNESS WHEREOF, I have hereunto set my hands this 9th day
