Thank you for your recent application for credit privileges which we have
carefully reviewed. We appreciate your interest in doing business with our
company; unfortunately we are not able to approve your application at this
time yet we would still like to work with you through a cash basis program.

In regards to your application it was denied due to the fact your company does
not have a sufficiently long credit history. Here at Wells & Associates, LLP we
require at least five years worth of positive reports for all new business line or
credit customers.

Our cash basis program is a program offered for six months where we would
keep track of cash receipts and disbursements made from you through a cash
on delivery mailing service. You would simply mail in your cash receipts for our
company to monitor. After the six months is up, we do encourage you to
reapply for credit privileges. We have also assigned you a customer

included with the application. This form is needed in order to move forward
