TO ALLOW MEDICAL EXAMINATION OF
THE ACCUSED
Accused, through the undersigned Public Attorney, unto this
HONORABLE COURT, most respectfully states that:

1. Above-named Accused, is presently locked up at the PNP

Lock-Up Cell, x;
2. The Accused was reportedly experiencing hypertension while
confined inside his cell;
3. Due to this, he is experiencing chest and nape pains as well
as other similar symptoms.

WHEREFORE, for humanitarian considerations, it is most

respectfully prayed before this HONORABLE COURT that Accused
be allowed to undergo medical examination at x District Hospital,
x City on April 30, 2019, Tuesday, around 10:00 o’clock in the
morning, subject to security rules and regulations of the BJMP, x
City.
