Title : Electric Shock and fall down

What Happened?

 The plan for the day (AA.0B.YYYY) was to carry out barbed wire and danger board
fixing work for X - feeder outside the boundary wall which was not charged.
 Accordingly, a gang of 4 workmen of the general contractor reached the new
town Power Substation (PSS) at around 10.30 am.
 At that time Site Engineer didn’t reach at site. So the gang started barbed wire
fixing as per the plan.
 At around 11.30 a.m. Site Engineer reached at site and diverted the gang for
carrying out compliance of Site Observation points issued by the client
(dressing of HT cable, barbed wire and danger board fixing on 100 kVA DSS-
distribution station transformer) since client demanded the same for signing off
the Joint Measurement Sheet.
 So, site engineer took shutdown from sub-station operator verbally and
instructed the gang to carry out the activity.
 Shutdown taken from substation to DP structure inside the boundary wall
 The victim fixed the ladder with north side pole of DP structure with the help of
another workmen and was climbing on the same.
 After reaching on the isolator fixing channel, he was supposed to anchor the Full
body harness hook to the pole.
 During this fixing process, the victim spread his right hand which suddenly
touched the nearby charged rabbit conductor which was coming from (outside
the boundary wall ) Y – feeder(11kV) line from Old town PSS and connecting the
station transformer
 After getting electric shock he fell down to ground.

Causes of the Incident


 Casual attitude of supervising staff and lack of knowledge in regard to
incoming circuit.
 Violation of Cardinal rule -2. Bypassing the Shutdown procedure (PTP was not
prepared, Battery limit was not properly identified, Discharge rod, HMIT were
not used, Line clearance was not taken)
 Failure in identifying the charged line in the Station Transformer which was
connected with other feeder coming from Old town PSS
 Unplanned activity

Recommendations and Learning Points


 The Daily activity plan must be followed strictly.
 All the safety gadgets must be arranged at site before engaging workman
 Shutdown procedure (PTP must be prepared, Battery limit must be properly
identified, Discharge rod, HMIT must be used, written Line clearance must be
taken, Isolation of charge line must be done) must be followed strictly.
 Task specific pep talk to be conducted prior to start the activity.
 Workman should be allowed to start work after cross verification of battery
limit with induction tester and at spike rod point of discharge rod.
