my SSS UMID ID with No. 006-2366595-8 from Counter No. 3. I had a withdrawal
transaction yesterday dated March 28, 2019 around 12:45 PM under Counter No. 3, and I
forgot to get the said ID that I presented for the transaction.

I could not get it personally because of work commitment.

I am hoping for your positive response on this matter.
