ORDER NO. 50-140102-2196 (Duly Certified by the College Registrar)”, as
part of the requirements of Commission on Higher Education in getting a CAV.

However, I cannot personally claim my Letter of Confirmation and the

Photocopy of the Special Order No. since I’m currently working in Surallah, South
Cotabato, Philippines. Hence, I attached the authorization letter where I authorized
“RHEA MAE A. ALISER”, my cousin in getting the Letter of Confirmation on
my behalf. Also I attached my Professional Identification card bearing a no.
1900996 for your reference of my identity.
