Christian Greetings !

This is to request Authority to Travel of yours truly, Teacher in Charge of Mabuga Elementary
School , Division of Gapan City,Gapan North District to go at DEPED Central Office, DepEd
Complex, Meralco Ave, Pasig, Metro Manila for the purpose of getting the books and pencils
donated by Usec. Tonisito M.C. Umali, Esq.,Undersecretary for Legislative Liaison Office,
External Partnerships Service and School Sports on April 2, 2019.

Last time we don’t expect that there were many books and pencils they will gave to us, that’s why
we only used a car. In our surprise 19 big boxes of books were ready to be given to our
school….only 13 big boxes were being loaded to the barangay foton service car…3 big boxes of
pencils were not carried because of the lack of space and cannot bring the heavy weight of all the
remaining 6 boxes of books and 3 big boxes of pencils…each big box contains almost 3,000
pencils…Our school will be very blessed if were given the second chance to travel and get those
all donations.

Note: Teacher in Charge, Hazel A. Dela Peña will go also with his husband to help her carry the
remaining donations and like the last time we seek again for the help of the Barangay Captain
Honorable Anselmo G. Cabatuando and being a supportive Barangay Captain, He gave his
permission to use the car (FOTON), service of Barangay Mabuga, Gapan City and let their driver
to accompany and drive for us for the second time around.
