AFFIDAVIT OF UNDERTAKING

I, MARIA DIMAGAPI, of legal age, single, Filipino citizen, and a

resident of Bgy. Central, Diliman, Quezon City, after having been sworn
to in accordance with law do hereby depose and state: THAT-

1. I was appointed as ADMINISTRATIVE ASSISTANT 1 at the Office of

the City Administrator of the Quezon City Government;

2. Relative to the said appointment is the submission of credentials,

amongst of which are the Transcript of Records (TOR) and Diploma;

3. I have secured said documents from my graduate school, Philippine

Normal University, however, the issuance and release of my Transcript
and Diploma are pending to date per school’s policy;

4. In view of foregoing circumstance, I hereby undertake to submit my

Transcript of Records (ToR) and Diploma in compliance of the
requirements of the Office of the City Personnel of Quezon City once I
have obtained copies of said TOR and diploma;

5. I have submitted all requirements relative to the said appointment,

except my Transcript of Records and Diploma for reason afore-stated;

6. I have executed this Affidavit to attest to the truthfulness and

veracity of all my foregoing statements and for whatever legal intents and
legitimate purposes it may serve, especially in support for the processing
and release of my salaries, pending the submission of the said Transcript
of Records and Diploma.

IN WITNESS WHEREOF, I have hereunto set my hand and affixed my
