Dear Mr. Escario,

I am writing this letter to apologize for failing to participate in the online faculty evaluation for
nd
the 2 semester of S.Y. 2018-2019. I am fully aware of the ongoing faculty evaluation last
semester but I overlooked to participate in it.

The reason that I was not able to participate in the said evaluation is because in preparation for
our MOCK BOARD EXAMINATION last March 2 & 3, 2019 and March 9 & 10, 2019 we are
given a week to prepare and study prior to the examination. Most of us, BSA students, are
spending much time studying at home, study centers or even coffee shops. Due to some priority
issues, I decided to do the evaluation on the last day during our laboratory and yet it slipped into
my mind to do such on that day. I know as a student of this Institution it is my responsibility and
to abide with the rules.

I really understand the importance of the online faculty evaluation to the management and
humbly accept the consequences thereof.
