ABSTRACT
Background: Cutaneous tuberculosis constitutes 1.5% of extra pulmonary tuberculosis and the disease continues to
be a challenging one because of its multifaceted presentation. The present study was done to document the most
common type of cutaneous tuberculosis, atypical presentations if any and response to directly observed therapy short
course (DOTS).
Methods: All patients with clinically suspected cutaneous tuberculosis attending outpatient department of
dermatology in our hospital from October 2012 to April 2016 were included in the study. A detailed history of
presenting illness and thorough general, systemic and cutaneous examination was carried out along with
documentation of demographic details. Routine blood invetigations, biopsy and mantoux test were done. Diagnosed
cases were treated with DOTS.
Results: A total of 25 cases of cutaneous tuberculosis were included in the study. Most common type of cutaneous
tuberculosis was lupus vulgaris. Atypical presentations noted during the study were multifocal lupus vulgaris (LV),
co-existence of tuberculosis verrucosa cutis (TVC) and LV, TVC of lower lip, erythema induratum of bazin presenting
as annular plaque in one case and as erythema nodosum in another case. DOTS were effective in majority of the
patients.
Conclusions: Cutaneous tuberculosis is multifaceted. High clinical suspicion is necessary in rare presentations.
Coexistence of two or more morphological patterns can occur. Doubtful cases, 5-6weeks of therapeutic trail helps.
Adequate dose is essential for good response. Second line drugs are to be considered in case of failure /clinical
resistance.

Metode
Pasien yang memiliki suspek klinis TB kutis yang tercatat pada rawat jalan pada departemen
dermatology di rumah sakit pada Oktober 2012 sampai April 2016 termasuk dalam penelitian ini.
Riwayat lengkap penyakit dan pemeriksaan cermat menyeluruh, sistemik dan kulit di lakukan
bersamaan dengan dokumentasi rincian demografis. Pemeriksaan darah rutin,ELISA untuk HIV,
pemeriksan foto thorax, mantoux test dan biopsy telah dilakukan pada semua kasus. Pemeriksaan
sputum untuk basil tahan asam (BTA), pemeriksaan biopsi jarum halus kelenjar getah bening dan
pemeriksaan radiologi lain telah dilakukan pada kasus yang berhubungan.
Kasus yang terdiagnosis diberikan pengobatan DOTS selama 6 bulan dan respon pengobatan dinilai
selama 6 minggu dan pada akhir pengobatan, pencatatan efek samping selama pengobatan juga
dilakukan bila ditemukan selama pengobatan dilakukan.
Hasil
Total 25 kasus dengan TB kutis dimasukan ke dalam penelitian ini. Perbandingan pria dan wanita
1,5:1. Usia populasi pada penelitian ini bervariasi antara 5-40 tahun dengan usia rata-rata 25 tahun 6
bulan. Tempat predileksi yang sering adalah ekstremitas bawah yang terdapat pada 13 kasus (52%).
Tipe manifestasi klinis TB kutis yang sering adalah tipe lupus vulgaris yang terdapat pada 11 kasus
(44%) dan tipe yang jarang adalah erythema induratum bazin yaitu sebanyak 2 kasus (8%). Detailnya
tersedia di tabel 1.
Kasus yang disertai HIV ditemukan pasa 3 kasus (12%). Manifestasi yang tidak khas yang ditemukan
pada penelitian ini adalah lupus vulgaris yang multifokal, adanya tuberkulosis kutis verukosa disertai
lupus vulgaris, tuberkulosis kutis verukosa pada bibir bawah, eritema induratum bazin yang berbentuk
plak anular pada satu kasus dan berbentuk eritema nodusum pada kasus lainnya seperti pada gambar
1-5. Tes mantoux positif pada 84,2% kasus dan gambaran histopatologi yang khas ditemukan pada
90% kasus. Tabel 2 memperlihatkan detail penemuan laboratorium pada studi populasi. DOTS efektif
pada semua kasus kecuali pada 2 pasien (8%) dan akne trunkal diamati pada 2 pasien (8%) seperti
pada gambar 6 dan 7.
Diskusi
Pada penelitian ini, TB kutis mempunyai insiden yang tinggi pada laki-laki, hal ini mirip dengan
penelitian lainnya di India. Hal ini dapat dikarenakan tingginya angka risiko cedera pada laki-laki,
dimana pada penelitian ini kebanyakan pasiennya adalah pekerja berat. Kebanyakan pasien penelitian

TB kutis yang terbanyak adalah lupus vulgaris (44%), hal ini mirip dengan sedikit penelitian.

ditemukan. Terdapat detailnya di tabel 3.
Kami menemukan ekstremitas bawah menjadi tempat tersering , hal ini juga ditemukan oleh beberapa
peneliti. Tingginya insiden TB kutis pada ekstremitas bawah di India, disebabkan karena re-inokulasi
tuberkulosis basil melalui trauma minor, khususnya saat jongkok.
TB kutis awalnya diistilahkan oleh Pillsbury, Shelly dan Kligman sebagai, "di kulit tuberkulosis
memberikan gambaran dirinya dalam bentuk yang bervariasi dan mengherankan". Secara bersamaan,
kami menemukan banyak bentuk yang tidak khas pada penelitian ini. Lupus vulgaris multifokal
ditemukan pada laki-laki usia 40 tahun, terdapat sedikit penelitian yang melaporkan penyakit yang
multifokal di literatur. Keterlibatan multifokal biasanya terlihat pada pasien yang tidak divaksinasi
dan pasien yang kurang gizi, serta biasanya hasil mantouxnya negatif. Terdapat beberapa laporan
yang menemukan dua bentuk pada satu orang, dengan kombinasi TB kutis veruka dan skrofuloderma
menjadi yang terbanyak dilaporkan, termasuk penelitian ini. Penelitian imunohistologi dari granuloma
di TB kutis menunjukkan perubahan spektrum yang dibuktikan melalui rasio CD4 CD8 contohnya
lupus vulgaris dengan imunitas kuat, TB kutis veruka dengan imunitas menengah dan skrofuloderma
dengan imunitas yang rendah. Tergantung pada level imunitas seseorang pada periode waktu tertentu,
ekspresi TB kutis dapat berbeda. Alasan lain yang mungkin adalah TB kutis veruka dapat berkembang
dari auto inokulasi basil dari TB kutis berdekatan yaitu Skrofuloderma. Kami menemukan TB kutis
veruka di bibir bawah pada perempuan berusia 40 tahun yang merupakan tempat yang jarang,

dilakukan
oleh Maharaja dkk pada penelitiannya tentang bentuk klinikohistopatologi dari nodul nyeri tekan
eritem yang dominan terjadi di ekstremitas, dimana ketidakselarasan histologi terjadi pada 8 kasus
(total 30 kasus) dan diantaranya terdapat 3 kasus yang sugestif eritema induratum bazin. Bentuk
eritema induratum bazin yang tidak khas lainnya ditemukan pada penelitian ini dengan bentuk plak
anular di atas paha pada laki laki usia 25 tahun. Sehingga eritema induratum bazin tidak selalu dalam
bentuk yang khas seperti yang dijelaskan (nodul ulseratif di betis), histopatologi membantu dalam
diagnosis definitif dan diagnosis bandingnya.
Perubahan histopatologis yang khas terlihat pada 90% kasus yang mirip dengan penelitian di India
lainnya. Durasi terapi antitubercular [ATT] untuk tuberkulosis kulit berkisar antara 6-12 bulan dalam
studi yang berbeda. Kami menggunakan terapi DOTS yang diberikan selama 6 bulan

Durasi uji coba terapi dalam kasus dugaan TB kulit adalah 5-6 minggu, dengan pengecualian TB dan
pasien menunjukkan aktivitas klinis minimal sebelum pengobatan. Diagnosis digunakan untuk
ditinjau pada pasien yang tidak menanggapi saat ini.
Tetapi dengan munculnya multi-drug resistant tuberkulosis, pendekatan ini tidak dibenarkan. TBC
kulit MDR harus selalu diingat dalam pengelolaan pasien dengan kurangnya respon klinis terhadap
obat ATT lini pertama atau pada pasien yang menunjukkan penurunan klinis bahkan saat
menggunakan ATT. Diagnosis pasti tuberkulosis MDR sulit karena buruk tingkat isolasi dan
sensitivitas rendah dari tes diagnostik molekuler. Jadi, selalu dibenarkan untuk memberikan uji coba
ATT lini kedua untuk setidaknya dua bulan sebelum memberi label pada pasien yang tidak responsif
terhadap terapi.
Kami memiliki satu pasien yang baik (tinggi 6 kaki, berat 120 kg) pasien pria berusia 40 tahun dengan
skrofuloderma dikonfirmasi biopsi yang belum menanggapi dosis rutin ATT [kit AKT-4] bahkan
setelah 5 minggu terapi, berpikir dalam hal resistensi obat sebelum mulai agen lini kedua, kami
memberinya agen lini pertama yang menyesuaikan dengan per kg berat badannya. Dalam waktu 2
minggu setelah peningkatan dosis pasien merespons dengan segera. Oleh karena itu, dosis obat yang
memadai terutama disesuaikan dengan per-kg berat badan seseorang juga paling penting sebelum
mempertimbangkan diagnosis alternatif atau resistensi obat.
