CONTENTS OF PORTFOLIO

1. History
2. Examination
3. Summary of problems
4. Management of problems enumerated (investigations, interventions, outcomes, advice)
5. Discussion (basic sciences, management of case, reflections on evidence based medicine or lack of,
personal development and learning points from the case and its management, issues in
communication with the patient/family, peers, medico legal and ethical issues).
6. There should also be a brief reflection on the students’ experience with cases wherever relevant.

LAY OUT OF THE CASES IN THE PORTFOLIO(INFORMATION TO STUDENTS):

The first page should contain:


 CLINICAL CASE PORTFOLIO OF …………………(name of student and BMS no) presented for the Final
Professional Examination of The Faculty of Medicine MAHSA University Academic Session (year).
 Contents -Cases
 All cases are to be hand written in clear legible script.
 Use only blue or black colour pens.
 All cases must be in the English Language.
 Your name and matriculation number, date of clerking and module must be on the 1 st page of each
case.
 Use ruled A4 paper.
 The key clinical details must not exceed 3 pages.
 The discussion must not exceed 4 pages.
 Total number of pages should not exceed 7 for a case.
 List the References separately, you are advised to not exceed 5 references per case.
 You are advised to use the Harvard Referencing System for References.
 Use initials for the patient identity.
 You may use accepted abbreviations.
 The case and discussion may be presented in the form of a clinical presentation and this is preferred to
formal prose.
 You may use diagrams and tables.
 YOU MUST NOT EXCEED THE PAGE COUNT.
 Violations will be penalised.
