AFFIDAVIT OF WAIVER

I, SHIEVA MARIE GRACE L. PARBO, of legal age, single, Filipino and with residence at
Malandag, Malungon Sarangani Province having been duly sworn to in accordance with
law, hereby depose and state to wit:

That I will undergo on the job training at Gentuna, Tambler, General Santos City
starting January , 2019.

That by this affidavit of waiver, I do hereby waive my rights to claim damages and
or other money claims in case of death or injury that may occur on my part while in the
