An estimate of routine fall in haemoglobin levels after tubeless and totally

tubeless pcnl

Abstract

Aims and objectives


Aim of the study is to compare the average fall in haemoglobin levels in patients who
underwent tubeless & totally tubeless pcnl.

Material and methods


A total of 80 patients who were candidates for pcnl with no comorbids were randomized
into two groups;40 patients underwent tubeless in whom a 4.8fr DJ stent was placed at the
end of the procedure and 40 patients underwent totally tubeless pcnl in whom a 5fr ureteric
catheter was placed and kept for 24hrs.preoperative and postoperative HB was recorded
and compared for each group.

Results
From November 2017 to October 2018 eighty patients underwent pcnl, divided into two
groups. Tubeless pcnl (n=40) and totally tubeless pcnl (n=40). Mean age,mean stone size
and BMI were comparable among the two groups. Average fall in HB in patients with pcnl
was (mean 2.9g/dl). It was slightly more in patients with totally tubeless pcnl
(mean=3.17g/dl) as compare to patients with tubeless pcnl (mean=2.63). Blood transfusion
was required in 4 patients(10%) in totally tubeless group as compare to 5%(2 patients) in
tubeless group.

Conclusion
Pcnl however a safe procedure is associated with mild to moderate fall in postoperative HB.
When compared among tubeless and totally tubeless forms totally tubeless pcnl is
associated with a slightly higher percentage of average fall in HB and tubeless pcnl seems to
be safe regarding postoperative fall in HB. These results need to be studied further in detail
and on a large scale to find out the cause of this difference.
