AFFIDAVIT OF LOSS
I, RODOLFO L. CALALAY, of legal age, married , Filipino and resident of No. ___ Judge Juan
Luna Compound, Bgy. Paltok, Quezon City, after having been duly sworn to in accordance with
law, depose and say:

1. That I am the Campaign Manager of my brother Tanie Joe L. Calalay who ran for City
Councilor of Quezon City’s First District during the recently concluded Mid Term Elections
on May 13, 2019;

2. That part of the campaign materials of my brother were campaign flyers which I had
caused to be printed at One Digital Place, located at UP Shopping Center worth
P18,000.00 for 30 reams.

3. That however, due to voluminous work load and files the Official Receipt issued by the
printer was misplaced and lost, and was only discovered yesterday while I am preparing
the Statement of Campaign Expenditures (SOCE) as required by law;

4. That despite diligent efforts to locate the same all proved futile and in vain, until I was
constrained to consider the said fficial receipt as irretrievably lost;

5. That I am executing this affidavit to attest to the veracity of the foregoing loss and in order
to report the same with the Commission on Elections in lieu of the attachment required to
support the SOCE of my brother Tanie Joe L. Calalay;
