Pharmacy located at San Jose St., Pototan, Iloilo and owned by Mrs. Imelda P. Lico. This is

after the previous pharmacist has turned over to me the responsibility of becoming the

Pharmacovigilance pharmacist of the above mentioned pharmacy.

With this, I am held accountable to perform the tasks of a pharmacovigilance officer.
