In the exigency of the service, you are hereby designated as Youth Development
Session (YDS) Coordinator of SERGIA SORIANO ESTEBAN INTEGRATED SCHOOL
SPED CENTER effective June 4, 2018 to May 31, 2019.

This designationintends to assist the School Head in the implementation of the YDS hence,
bears the duties and responsibilities as follows:

1. determines, in coordination with the 4Ps Coordinator, the beneficiaries who will
participate in the YDS;
2. maintains a file of the participants for reference;
3. requests/invitesGuidance Counselors/Values EducationTeachers as resource
persons to ensure regular conduct of in-schools YDS with Pantawid youth
beneficiaries;
4. submitsthe schedule of the YDS to the Division Office for monitoring and evaluation
of the Division 4Ps Focal Person;
5. ensures at least once a monthactivity with 2-3 hours of interactive discussion and
conduct of a follow-up session within the month as deemed needed and agreed upon
by the beneficiaries;
6. ensures a copy of the attendance sheet during the sessions;
7. collaborates with DSWD City/Municipal Links and Division 4Ps Focal Person on YDS
implementation;
8. submits the YDS Documentation Form/Tool every first week of the quarter, and a
Year-End Accomplishment Report on the last week of April;
9. updates the Division Electronic Reporting System of the YDS regularly;
10. orients, in coordination with the 4Ps Coordinator, the parents and beneficiaries
regarding the YDS;
11. provides ongoing support, advocacy and monitoring of the 4Ps programs and
projects with special attention to YDS;
12. refersbeneficiaries to guidance counselors or appropriate personalities for
servicesbeyond scope where required; and
13. does other related tasks as needed.
