JOINT AFFIDAVIT OF UNDERTAKING

Philippines, both Filipino citizens, single, and of legal ages, after having been duly sworn
to in accordance with law, hereby depose and say, that:

1. We are the farmer-beneficiaries in the landholding covered by Transfer

Certificate of Title (TCT) No. CL-1234 of the Registry of Deeds for the City of Quezon
registered in the name of Name and Name, with an area of THIRTY-NINE THOUSAND
EIGHT HUNDRED SIXTY-FOUR (39,864) SQUARE METERS, more or less, located at
__________, Quezon City, Philippines;

2. The above-cited portion of land was included in the coverage of the

Comprehensive Agrarian Reform Program (ARP) through Direct Land Transfer (DLT)
pursuant to Republic Act No. 6657;

3. We the undersigned hereby undertake to keep the said landholding under

co-ownership;

4. We are hereby executing this Affidavit of Undertaking freely and voluntarily

in order to attest to the truth of the foregoing facts as a requirement of the Department of
Agrarian Reform, to bind ourselves to the foregoing commitment, and for such other legal
purposes this instrument may serve best.
