Cadu-an, Cadiz City, Negros Occidental after having sworn to in accordance with law,
voluntarily depose and say;

1. That I am an applicant for Industrial Sand and Gravel Project at Brgy.

Caduha-an, Cadiz City, Negros Occidental.
2. That no complaints have been filed in the office of the barangay and other
concerned agencies with regards to my application for Industrial Sand and
Gravel Project.
