Student Teaching Evaluation Form - Mid-term


As we get started, Cooperating Teacher
what is your role?

To complete this form, you may need to know:

The Student Teacher's name and their student number - it starts with an E and is a total of
nine characters long.

The Student Teacher Supervisor name and their EMU e-mail address, ending in @emich.edu.
(For FERPA reasons, please do not use a non-EMU e-mail address.)

The Cooperating Teacher's name and school/district e-mail address. (For FERPA reasons,
please do not use a personal e-mail address.)

This form takes between ve and ten minutes to answer the questions - somewhat longer if you are
adding comments. It must be completed in one session; it is not possible to save and return to a
partially completed evaluation at a later date.

If you wish to have a "working" copy, with all of the questions, please refer to the Student Teaching
Handbook.
