SUBJECT : PHIL - IRI FORM AND SUBMISSION OF REPORT

DATE August 8, 2018

1. The submission of Phil-IRI Group Screening Test (GST) result was scheduled in
July 2018 but only seventeen (17) districts have submitted using different forms.

2. This division will use the attached format for uniformity until another form might be
required to provide necessary data for regional reports in the future.

3. Each district must have a consolidated report to be submitted to EPS Dela Vega
for English and to EPS Besinio for Filipino on or before August 15. 2018.

4. Travel and other incidental expenses relative to this activity shall be charged to
school MOOE, PTA fund and other local funds subject to usual accounting &
auditing rules and regulations.

5. For guidance and compliance of all concerned.
