That I, RUEL ACASIO, Filipino citizen, married, a resident of Davao City, after having been
duly sworn to in accordance with law, hereby depose and say that:

1. I am the Department Head of the Motorpool Department of the Davao Light & Power
Company, a corporation duly organized and incorporated under the Philippine laws.
2. I am charged with all the Company’s motor vehicles and the supervision of the use
thereof.

5. The color of the said motor vehicle was changed from its original color to White;
6. The purpose of this affidavit is to support our request for the correction of entry
(Color) in the certificate of registration of the above said motor vehicle from COLOR
NOT INDICATED to WHITE.
7. Further, I am executing this affidavit for the aforesaid purpose as well as for attesting
to the truth of the statements herein declared.
