AFFIDAVIT OF GUARDIANSHIP

I, CLARITA O. CALDERON, of legal age, married, Filipino citizen, and

presently residing at Barangay Aridowen, Sta. Teresita, Cagayan, after having been
duly sworn to in accordance with law, do hereby depose and say:

1. That I am the grandmother and guardian of KRISTEL JOY C. BAJE, born on

November 22, 2000 to Erwil S. Baje and Gina C. Baje at Gonzaga, Cagayan,
who have an interest in applying for a Fisheries Scholarship Program (FSP) of
the Bureau of Fisheries and Aquatic Resources (BFAR);

2. That the above mentioned applicant is under my care and custody;

3. That I am competent to certify in behalf of her parents in her application for

FSP and for whatever legal purpose;

4. That I am not an imbecile, insane, a vagrant or vicious person or a habitual

drunkard or a habitual criminal, and have abandoned, neglected or refused
to support said applicant or caused her to commit offenses against the law;

5. That I am executing this affidavit to attest to the truth of all the foregoing
statements and for whatever legal purpose it may serve.
