with law, hereby deposes and states THAT:

High School (Integrated Junior & Senior High School),
Diffun, Quirino of Department of Education, Schools
Division Office of Quirino;
2. That by means of this Affidavit, I hereby AUTHORIZE
the Department of Education (Deped) particularly at the
Division Office of Cabarroguis, Quirino or its duly
authorize representative to deduct from my Terminal
Leave Benefits all my financial obligations if any;
3. That I am executing this Affidavit freely and voluntary
and to attest to the truth of the foregoing facts and for
whatever legal purpose or intent this may duly serve.
