What is an affidavit of loss?

An Affidavit of Loss is a written sworn statement describing the particulars and circumstances of the
physical loss of securities which includes legal documents like IDs, Passport, Drivers License, certificates,
issued by government agencies, colleges, and private organizations.

Physical loss of the items may be due to theft, or destruction by flood or fire. The affidavit must contain
the following information such as the owner's name, address and the circumstances how the loss
incurred.

These affidavits are needed to obtain replacement copies of the lost items.
