 Two (2) fully accomplished copies of Personal Data Sheet (CSC Form 212) and Work Experience Sheet with latest passport size picture
(CSC Form 212 can be downloaded from csc.gov.ph website);
 Resume

 Diploma

 Transcript of Records and Certificate of Graduation

 Certificate and modules of trainings and seminars (please submit only documents relevant for the position being applied for.

Online trainings may not be considered)


 Certificate/s of Eligibility or Board/Bar Rating

 Certifications/Office Orders of current and previous additional designations, if any

 Employment/Service Record along with current and previous job descriptions which are issued and certified only by authorized

human resource personnel and/or supervisors (please submit only documents relevant for the position being applied for)
 Appointment Paper if previously/currently working in government service

 Other documents supporting information in application and resume

East Avenue, Diliman, Quezon City or at your nearest SSS branch office for Non-NCR applicants.
