LAKEVIEW BAND AND ORCHESTRA CONTRACT

the band website.

Student and parent/guardian have reviewed the 2019-2020 Lakeview Middle School Band / Orchestra Calendar.

Student and parent/guardian have printed and signed the OCPS Parent Technology, Media, and Information
Consent Forms located on the band website.

rehearsal etiquette basics. I understand the locker room expectations and will abide by the rules and procedures. I understand
the consequences of my actions if I choose to not abide by them.

day in band include: my well-­­maintained and operational instrument, 1” black band binder containing all of my issued music,
and multiple pencils.

the calendar on the band website (www.lakeviewmsband.weebly.com) is a live document which dates are subject to change
without notice. I understand that I must be proactive and check the online calendar for all events throughout the 2019-2020
school year.
